The ranks

Short blurbs only. Titles below are the classic vocabulary used in history and cultural discussion of American Cosa Nostra — not a brand ladder.

Top of the family

Boss

The head of a family. In popular lore sometimes called “capo dei capi,” but historically that title was rare / contested — most accurate: Boss of a family. Final say on major decisions; the public face of authority in the hierarchy myth.

Second chair

Underboss

Number two. Runs day-to-day when the Boss is unavailable; often the presumed successor in the cultural story. Reports up; coordinates down.

Counsel

Consigliere

Trusted advisor — not usually a line commander. Mediates disputes, offers counsel, sits beside power rather than under every order. The “wise ear” of the organigram.

Middle management

Caporegime / Capo

Crew captain. Runs a crew of soldiers (and often associates). Bridge between the top and the street-level membership in the classic model.

Made man

Soldier

Initiated member — the “made” man of lore. Full membership status in cultural accounts, as opposed to hangers-on. Reports to a Capo.

Outer ring

Associate

Works with the group but is not initiated. Can be highly useful; still outside the formal made-man circle in traditional ranking talk.

Reality check. Real families, decades, and cities differed. Charts like this are a teaching device — encyclopedias and historians stress variation. Movies flatten it into a neat org chart.
